What is Exercise Rehabilitation?
Exercise rehabilitation is a structured, progressive programme of exercises prescribed specifically for your condition — not a generic handout of stretches, but a plan built around your injury, current strength and movement capacity, and stage of recovery. It’s the most evidence-based tool in physiotherapy for building a lasting result: while other treatments can reduce pain and restore movement in the short term, appropriately dosed exercise is what rebuilds the strength and tissue capacity needed to prevent a problem recurring once the acute symptoms have settled.
How It Works
Tissue — whether muscle, tendon, or the structures surrounding a joint — adapts to the load placed on it. Too little load and it gradually weakens; too much, too soon, and it can be aggravated or re-injured. Exercise rehabilitation works on the principle of progressive overload: applying a dose of load that’s enough to stimulate an adaptive response — strengthening muscle fibres, increasing tendon capacity, or improving the neuromuscular control around a joint — without exceeding what the tissue can currently tolerate.
This means the “right” exercise changes as you recover. Early-stage rehabilitation after an injury or surgery often starts with restoring basic range of motion and gentle activation of the surrounding muscles. As symptoms settle and capacity improves, load is progressively increased — through resistance, range, speed or complexity — moving through stages until you’re back to the strength, control and confidence needed for daily activity, sport, or work.

Exercise rehabilitation is a central part of Andrew’s treatment plan for nearly every condition he sees. It’s used to rebuild quadriceps and hamstring strength and joint control after an ACL injury, restore capacity in tendons affected by conditions like Achilles tendonitis, and address the movement patterns and core control that often contribute to low back pain. It’s just as important for guiding staged strengthening during post-operative recovery within the limits set by your surgeon, managing chronic joint conditions such as osteoarthritis where appropriately dosed strengthening can support joint function, and building the capacity needed to safely return to activity after a general sport injury.
Because nearly every musculoskeletal condition eventually needs some degree of loading to fully resolve, exercise rehabilitation is rarely delivered in isolation — it typically follows, or runs alongside, manual therapy, dry needling or other techniques that address pain and mobility in the earlier stages.
Exercise Rehabilitation vs “Just Exercising More”
A common misconception is that any exercise, or a generic set of stretches, will do the job. What distinguishes exercise rehabilitation is the dosing and progression: the specific exercises, load, range and frequency are chosen based on your injury and current capacity, and are progressed according to clear, staged criteria rather than a fixed timeline. Too little load and progress stalls; too much, too soon, and symptoms flare up. Finding and adjusting that level throughout your recovery is difficult to judge accurately without professional guidance, particularly after surgery or a significant injury — which is Andrew’s role in your programme.
What to Expect During a Session
Andrew will assess your current strength, movement and function relative to what’s needed for your goals, then design a programme addressing the specific deficits found. In clinic, this typically involves being taught the correct technique for each exercise, working through the appropriate load or resistance, and adjusting based on how your body responds. Sessions also serve as checkpoints — your programme is reviewed and progressed at appropriate points as your strength and control improve.
Aftercare
A degree of muscle soreness in the 24 to 48 hours after a session — similar to how a muscle feels after starting a new gym programme — is a normal response to appropriately dosed exercise and isn’t a sign of harm. This is different from a sharp increase in your original joint or injury pain, which is a signal to ease off and check in with Andrew before continuing. A consistent home exercise programme between clinic visits is usually essential for lasting progress, since the tissue adaptation that drives recovery depends on regular, appropriately dosed loading, not just what happens in the clinic.
Is Exercise Rehabilitation Right for You?
Exercise rehabilitation is appropriate for the great majority of people and conditions Andrew sees, and is generally considered very safe when properly dosed and progressed. Extra caution and modified programming are needed around acute, unstable fractures, in the early stages after surgery where your surgeon has set specific movement precautions or timeframes, and where an underlying medical condition — such as an unstable cardiac condition — needs to be factored into how much load and intensity is appropriate. If a flare-up of severe, unmanaged pain needs to settle first, Andrew will address that with other treatment before building up the exercise side of your programme. He’ll always tailor the starting point and progression to what’s safe and appropriate for you.
